The item is a drop which only one member of the raid can get, once a member has compelted the quest once they will not get credited with it again if they obtain a second head.

The item shares the same qualities as the items off other end pre bc raid bosses such as the Heart of Hakkar, off Hakkar in Zul'Gurub, the Head of Onyxia in Onxyia's Lair, the head of ossirian, off ossirian in AQ20 and the Eye of C'Thun from C'thun in AQ40

BC type equivalents of this are the Blood of Zul'Jin off Zul'Jin in Zul'Aman or the Verdant Sphere off Kael'Thas in Tempest Keep.

EDIT: Going to remove the 'Gland off Kel'thuzad' part...speculating it wont be the same once the level 80 Naxx is implemented.
