Where you hear it

igquestfailed.ogg is the deflating sting that accompanies the red 'Quest failed' text. Escort quests are the usual cause: the NPC you were protecting walks into a pack, dies, and the quest resets. Timed quests and some event chains can fail the same way. Because it is brief and unmistakably negative, the sound reads as a joke 'nope' in voice chat. Players who failed the same escort three times tend to remember it vividly.

TriggerPlays when a quest in your log is marked Failed, typically after an escort NPC dies or a timed quest expires.

Notification sound

Over a text, a 1.2-second downward sting registers instantly and is hard to confuse with a stock phone chime.

Discord soundboard

On a soundboard the 1.2-second Quest Failed clip lands and finishes before anyone has to stop talking.

  1. Tap iPhone ringtone (.m4r) above and choose Download.
  2. Open the Files app, go to Downloads and open the file.
  3. Tap Share, then Use as Ringtone (iOS 26).
  4. Pick it in Settings, Sounds & Haptics, Ringtone or Text Tone.

  3. Name it and choose an emoji. You need the Manage Expressions permission.

Discord takes clips up to 5.2 seconds and 512 KB; this one fits.

Windows

  1. Windows sound schemes only take WAV files, so convert the MP3 to WAV first (Audacity or any audio converter does it in one export).
  2. Open Settings, System, Sound, More sound settings, then the Sounds tab.
  3. Pick an event such as Notification, click Browse, choose the WAV, then Apply.

OBS alert

  1. Download the MP3 above.
  2. In your alert tool (Streamlabs or StreamElements) set it as the alert sound file.
  3. Or in OBS add a Media Source with the MP3, untick Loop, and fire it with a hotkey.
