1. Normal Difficulty
2. Begin fight, go all out on boss, but save lust for after you fall.
3. You'll drop down and somehow survive a fall that would otherwise kill your knees, among another things.
4. Kite boss to a corner, lust, and nuke and kill asap.
5. If you're like me you may only get either a row or a column, and then have to try for the other one next week, that works too.
6. Boss dies.

This achievement is self-explanatory and can be done in a single kill.
Save Time Warp for phase 2 so that you can kill Fallen Avatar before he casts [spell=235572].
As an opening note, I have not done this achievement yet and will update after I have with any info that changed as we tried it.
Unbroken Diagonal was indeed removed from this achievement and is not longer needed.
Adding this diagram that I will reference later to help visualize what I mean.
When the Maiden of Valor dies and the boss break the platform and begins P2, the platform you land on (and miraculously survive a 300 ft drop to) is in the shape of a square. Throughout P1 and 2 the boss will cast Rupture Realities which does a ton of damage to everyone, reduced by distance from the boss. In P2 though, casting this will cause him to break the platform underneath his feet.
For this achievement you need to kill him without breaking an entire line, and an entire row, of spaces. What this means is that for Unbroken Row credit, you'd need to not break any of 1, 2, and 3 on the diagram OR not break any of 4, 5 and 6 OR not break any of 7, 8 and 9. Similarly for Unbroken Column credit, you'd need to not break any of 1, 4 and 7 or the other columns.
This achievement CAN be done in multiple kills, and is probably intended to be done this way. That having been said, with enough DPS it could be done in one kill.
The boss only casts Rupture Realities every 45 seconds. The push from P1 to P2 happens when the boss hits 100 energy, something you can delay for quite a while if done right. This means that in theory you could destroy a 2x2 square in any corner and still have both an Unbroken Row and an Unbroken Column. For example, if you destroyed 1, 2, 4 and 5 on the diagram, you would still have 3, 6, 7, 8 and 9 left, completing both criteria.
With that in mind, there are obviously still other mechanics in this phase, and giving yourself 2 narrow strips of platform to work with makes these hard to deal with, so I don't recommend this until you're in better gear.
