I believe the only titles that are permanent are the season's Gladiator titles. To elaborate from that, Challenger, Rival, Duelist, and Gladiator are all temporary titles that will be taken away at the start of the next season (unless you earn it again) whereas if you obtained this season's Grievous Gladiator title, it will be permanent and you will keep it forever (like Merciless Gladiator, Tyrannical Gladiator, etc.)

It has been like that yes, however it now awards a FoS with a title reward attached to it, you cant say indefinitely that they're only temporary without acknowledgment from a GM.
