When you complete the build of your level 3 gladiator's sanctum you must walk outside your front gates. I searched what to do and I was told to walk to the outside of highmaul. That is Kharg and he does not talk to you unless you proceed outside to talk to Gharg.No you can not miss Gharg. I hope this helps those who needs it.

When I won in the sanctum this quest was automatically completed, without killing 100 people.
Tagging a [class=6]'s ghouls from [spell=42650] will count towards your kills.
