In order to unlock the third relic slot on your weapon you also have to finish the quest A Hero's Weapon. All these quests have the same name: A Hero's Weapon.
I just completed this achievement on 11/25/2020 and can confirm you can still get these appearances in Shadowlands. I went to the Barber Shop to change my flight form appearance after earning the lunar wing form and I was pleasantly surprised by new cat and bear form appearances!
