Summon Elder Murk-Eye using a Mrgrglhjorn, then defeat him and Harjatan in Tomb of Sargeras on Normal difficulty or higher.

You'll need someone with the [item=143660] Toy in their collection in your raid to complete this, which is obtained by completing the No Shellfish Endeavor Achievement.
To spawn [npc=121071] someone in the raid has to use the [item=143660] toy which is obtained by completing the No Shellfish Endeavor achievement.
Use the toy at the start of the fight, focus [npc=121071], and after he dies you can kill the boss and you'll get the achievement.
