Upon reaching 66% and 33% health, Viz'aduum the Watcher will open a Demonic Portal to retreat to a nearby Legion Command Ship.

If the encounter resets at the final ship, and slowing dps in phase 2 or waiting 10 seconds to take the portal don't fix it, try changing your camera angle so you face away from him as you take the final portal. None of the other suggestions worked for me, and after about a dozen tries, I did the camera angle change and that worked for me.
Never mind I just figured it out. Apparently if you do too much damage to him before the next phase he bugs out. So if you do too much damage to him after he starts opening portal to phase 3 he despawns. Stop damaging him as soon as he starts channeling the portal and you should be fine!
