TheWoWDBJust FYI, if you forget to pick up the quest item, you can go retrieve it even after having done the last boss. Use the transporter located on the final boss platform and it will teleport you back to near the beginning hallway. If it is the Soulsever Blade you need it's just forward and to the left; if it's the Soulweave Vessel, then its a bit more of a hike forward into the hallway and all the way down the right side hallway where the first boss was. This item can only be looted from the heroic version of the instance and all dungeon quests from the Tavern/Inn now require you to kill the last boss in each instance also.
If you are as geared as you can be and/or don't really want to run heroics, you can also complete the dungeon on normal and still get credit for the boss kill required. You then just extend the heroic lockout and run back in on heroic to collect the item.
How does one actually get this quest now, since Teron'gor runs away and you don't actually slay him?

Description
You! You're the one that found me the blade, correct? I have need of your services again. The blade worked wonderfully, allowing me to sever the tormented undead from this world. Their screams were such beautiful music. However, the Soulsever Blade cannot be repaired, as it is partially woven with spirit energy that is drawn upon each time a soul is released. I need another, as there are still dark forces at work in this place, raising spirits and turning them.

Auchindoun is the holy mausoleum of the draenei, a sanctum of the Light in which the spirits of the dead find respite. The crystalline structure also serves to protect and shield draenei souls from their eternal enemies: the Burning Legion, who hunger eternally for draenei spirits.
