The buildings dont have to be up at the same time - atleast not for medium.
I started with trading post and barn, the barn was upgrade to level 3.
After this I replaced the barn with the frostwall tavern.
The tavern I replaced with the lumber mill and after I upgraded this to level 3 the achievement popped.

Bugged for me with small buildings
Anyone else having a problem with this when you evidently have two level three large buildings (me myself having a Mage tower and a gnomish gearworks) but do not get credit for them on the achievment?
