At 100+, make sure to let the boss move towards the cauldron a few steps, THEN nuke him. The game wouldn't give me the achievement for nuking him where he spawned.

I finally had a chance to go in the scenario and try for the achievement again and it is in fact working as intended!------------------------------------------
This may be bugged at the moment, or I am doing it wrong; twice now I have immediately picked up the orange and kept him away from it and no achievement.
Patch 5.0.5 Hotfixes - October 16:
- The achievement Monkey in the Middle can now be earned when defeating Captain Ook in the Unga Ingoo scenario.I've been running the scenario multiple times and finally got it, after fix.
