Looted a [item=268807] from a [npc=259721] during first stage of Void Assault "While We’re Down" in Amani'Zar Village (Zul'Aman). Completing the quest validated the associated criteria for the achievement.
Looted a [item=266448] on Hal'hadar tagged mobs ([npc=258746]/[npc=259065]/[npc=258745], don't know which one exactly gave me the item) during "Battery Rush: Downsteram" at the Broken Throne (Zul'Aman)
My guess for the two other items is that they will be lootable next week at Eversong Woods void events.As per the comments below, it looks like [item=268809] can be found in Ritual Site: Daggerspine Point and [item=268808] from some void enemies (confirmed in other comments : [npc=256315] during the Void Assault at the Temple of Jan’alai (Zul’Aman) and Void Slayers in Ritual Site: Daggerspine Point).
