To be able to summon Manifestation of Wrath you need wait until world quest Swarming Souls is up.
During the quest you need to deliver souls to Avowed Ritualist. The rare have a chance to spawn after a delivering.You need to deliver 6 souls to complete a task. But you can deliver then as many times as you want.
Just dont kill enough common manifestations to finish the quest and you will be good.
I tried three times: 1st - rare spawned on 9 soul delivered, 2nd - on 11th and 3rd - on 4th.TomTom coordinates:

As of patch 10.0.5, I can confirm it's not bugged.
I did the WQ with War Mode on and just kept turning in souls. I'd say it took me about 40 or so before it spawned. There were some other people there doing the WQ too.
Manifestation of Wrath is located south of the Sanguine Depths entrance in Revendreth.
It's available only during the Swarming Souls world quest.
To summon rare you have to bring [npc=169916] to [npc=169917].
It should take about 10 souls to spawn Manifestation of Wrath.TomTom coordinates:
