Got all 12 heirlooms and then it told me quest failed. Sitting on my sidebar saying 12/12.
Edit: left and came back and was given the weapon.

On This WQ You need to Loot 12 [item=258321], You can pick them up from [object=612932] or drop from mobs aroung.
