Can be completed just by killing Peroth'arn.
If Illidan disappears, it tells you that the crystals are locked and that you need a key.
I had to exit the instance and restart all instances, before he would return for me.
I think it's a rare glitch, as not many people seem to be complaining about it.
I was level 100 when I did this. So it might have been that I was killing too quickly.

Description
Between us and the Dragon Soul lies a legion of demons. They will have to be dispatched before we proceed. If my timing is correct - and it usually is - a young night elf named Illidan Stormrage should be crossing our path shortly. He will undoubtedly be of some use when it comes to demon hunting. Help Illidan disable the demons' portals, then slay their leader, Peroth'arn.
Getting there

Ten-thousand years in the past, the night elven capital city of Zin-Azshari was a magnificent, sparkling metropolis. Situated on the shores of the Well of Eternity, it was here that the aristocratic Highborne first opened a portal into the Twisting Nether and invited the Burning Legion to invade Azeroth.
