Making them as TomTom waypoints (I've included the Horde points as well, feel free to not add them, but half of them are virtually beside, so if you're aiming for the Desecration as well, here they are.)

(The one in the Swamp of Sorrows and Badlands are not necessary for the [FLAME WARDEN] achievement, but you might want to take them for the extra Burning Blossom.)

Fastest path I found to Honor/Extinguish at the same time was (use the coords above)

Fly to Zul'Aman
Run up to Eversong Woods, Extinguish the Flame at North Sanctum.
Head over to Silvermoon, Steal the Flame there.
South into the Ghostlands, Extinguish the Flame at Tranquillien.
Back to Zul'Aman, fly to Chillwind Point (Western Plaguelands), Honor the Flame there.
Mount up and ride into Tirisfal Glades, Extinguish at Brill.
Head over to the Undercity, Steal Flame.
South into Silverpine Forest, Extinguish at The Sepulcher.
East into Hillsbrad Foothills, Honor at Southshore, Extinguish at Tarren Mill.
North to the Hinterlands, Honor at Aerie Peak, Extinguish at Revantusk Village & ride back to Aerie Peak (long ride, sorry.. no short way)
Fly to Arathi Highlands, Honor at Refuge Pointe, Extinguish at Hammerfall.
Fly to Wetlands, Honor at Menethil Harbor.
Fly to Dun Morogh, Honor at Kharanos.
Fly to Loch Modan, Honor at Thelsamar.
Ride South into the Badlands, then West over to Kargath, Extinguish at Kargath.
West into Searing Gorge, cut through Blackrock Mountain, East to Flame Crest, Extinguish at Flame Crest.
South to Morgan's Vigil, Honor at Morgan's Vigil.
Fly to Redridge Mountains, Honor at Lakeshire.
Fly to Duskwood, Honor at Darkshire.
Fly to Blasted Lands, Honor at Nethergarde Keep.
Ride North into the Swamp of Sorrows, then East to Stonard, Extinguish at Stonard.
Back to Nethergarde Keep, fly to Booty Bay. Exit Booty Bay, Honor/Extinguish outside of Booty Bay (2 camps, Horde & Alliance) [Congrats on Extinguishing]
Back to Booty Bay, fly to Westfall, Honor at Sentinel Hill.
Fly to Stormwind, ride to Goldshire, Honor at Goldshire. [Congrats on Honoring]
