Someone Could Trip on These!

This achievement is a personal achievement, so everyone in the group has to click on each Toy individually. The toys can be clicked on in any order, you don't need to go from the first to the sixth. The achievement also carries over between runs, so it doesn't need to be completed in a single run if you're unable to for any reason. This achievement can also be completed after the last boss is killed as the toys don't despawn at all, if you're trying to do it solo and are in a group that don't want to wait for you.

You'll want to kill Mistcaller before attempting this achievement, as some of the rooms and passages between them will be blocked off by the mists if you try and do it beforehand.

The First Toy is the Super Happy Fun Ball, located at 52.5, 30.0
The Second Toy is the Creepy Doll, located at 63.5, 47.5
The Third Toy is the Bed Time Stories, located at 57.0, 42.0
The Fourth Toy is the Drum, located at 49.5, 29.0
The Fifth Toy is the Pan Flute, located at 58.5, 34.0
The Sixth Toy is the Harp, located at 69.0, 43.0

You aren't able to place waypoints via tomtom using the /way command because it's a dungeon, but by alt + right clicking on the map you can mark locations, and hovering the mouse over the waypoint shows the coordinate, if you zoom right in on you map and place a couple of markers in the general location of the ones in this screenshot of the map you'll probably be close enough in terms of coordinates.

Extra tip if you're struggling to see some of the smaller toys (such as the Pan Flute or the Super Happy Fun Ball): If you go to your graphics settings and turn "Ground Clutter" slider down to 1, they will be easier to see as there will be less shrubbery hiding them.

Keep in mind that you cant use the coordinates for TomTom as it is a dungeon, but you should have a fairly straight forward time if you are using this map to coordinate your route!
