In order to make him spawn you need to kill all other bosses in the dungeon, even when the doors don't really block your way.
If you are having trouble with him resetting, avoid tanking the boss near the side tunnels, as he will cast his teleport on you and the wraith adds will cause the event to reset as soon as you are teleported.
