Karthax is not inside the cave whose entrance is inconveniently located pretty much right where the WQ marker is on the map.
He is up above on a ledge.When I went to do the WQ, I got confused and ended up in the cave, only to be ambushed by a bunch of mad imps.
Don't be like me.If you don't have flying, you can follow these directions:
From the Watcher's Aerie flightpoint, follow the road downhill, through the port-like opening in the cliffs.
Continue towards the VotW dungeon entrance area then turn left after you pass the summon stone.
Keep following that road downhill and turn right where the road splits.
Now theres 3 possible paths, and you take the rightmost one.
Once you've passed those wooden poles, theres another 3 possible paths. Karthax is standing at the very end of the middle path.You have to get past quite a lot of spiders and a few hard-hitting elite mobs, and there is no way to avoid aggroing them.
If you have a spell like the rogue's vanish or the mage's invisibility, you can pop it after you've moved past them.
Drops a [item=140762] for warlocks on the hidden appearance quest The Grimoire of the First Necrolyte.
I finally figured out how to get on this ledge to fight Karthax and he is nowhere to be found.... is he still available to fight?
