In order to get credit for every location, you will need to be not just in the right spot but also at the right elevation.

For Nexus Spire you should head for the centre of the Nexus, where the white beam comes out of the ground.

For Surge Needle you need to be at about the same height, on the platform mid-air at the location indicated on the map. Do not go to the Focused Void at the bottom. You will aggro Voidtouched Channelers and not get credit.

For Nexus Foundation getting close to the bottom of the Nexus pillar should be enough. You do not have to enter it to get credit.

You should travel at once to Coldarra in Borean Tundra, {name}. There, I would recommend you scout out a few key locations and report back to me. I know the area well, and should be able to tell you how to proceed. Make haste!
