If you happen to end up in the wrong part of the map with no Prophet Velen when the vision starts (north side of Karabor, as happened to me), drop the quest and restart it, for there is no way to get to the place where the stages of the quest take place. Once dropped and re-picked up, you should be on the stairs on the easternmost part of Karabor where Prophet Velen actually is.
Edit: Several others have suggested letting the mobs kill you and resurrect on the opposite side of the door, but this did not work for me or two guild members. If it works for you, good on you, but I recommend everyone at least trying the drop-and-reacquire approach before resorting to such draconian measures.

This just happened to me, but after dropping the quest multiple times and re-logging, I was still spawning to the north of Karabor. To solve it, I let the mobs in the area kill me, then (in ghost form) I ran through the door at (77,41) and visited the kind Spirit Healer at (78,46). I was then able to kill Krull and complete the quest.
It's worth noting that Velen is at 35.3, 49.1 in Moonwillow Peak, not at the coordinates at the top of the page.
