Several people have mentioned this quest not working correctly. The most likely reason is that they're not beginning it at the right time. You have to start the quest right when the lion in the MGM logo roars, that way the lyrics will line up properly with the action.

For those with issues where Yrel and Velen are just standing there. After a few dozen times, found a way to get the quest to properly respawn. I found the main reason for the problem is I had flight, and was skipping around a bit to other quests, missed the main trigger. No matter what I did, I could not get the quest to trigger again, dropping several times, etc.
What I had to do was fly a long way from the entrance, way way out. If you think you're far enough, fly out a bit further. Then drop the quest.
Fly back to the main entrance and pick up the quest from Velen, then run on foot all the way the archway, no flying. run through the archway, up the hill, and Yrel should be standing part way up waiting for you, and away ya go.

    Rulkan's gift to you, the Fragment of Anguish, will allow you to pass into the fortress unhindered. Go up these stairs to the doorway, then use the fragment to enter the compound. Once you are inside, you will find three altars powering the magical barrier around their stronghold.
