Voodoo Zombies

I had a problem with sometimes not getting credit on my hunter. It seemed to me if I made sure to get the killing blow myself by putting my pet on passive before the zombie died, I consistently got credit. I also ran in to melee the last bit of damage to make sure that Bwemba was near the zombie when it died. That also might explain why you were sometimes getting credit for someone else's tagged zombie - perhaps you were getting the killing blow when that happened.
