This is shamans' quick, expensive heal.

In Cataclysm (as of 4.0.3a), all healers have the following types of heals:

Quick, expensive heal: 1.5 second cast, very high mana cost, high heal value.
Inexpensive heal: 3 second cast (usually talented to 2.5), very low mana cost, low heal value.
Slow, big heal: 3 second cast (usually talented to 2.5), high mana cost, very high heal value.

The mana costs differ very slightly between classes. Two of them have the quick heal cost more than the big heal.

Oh healing surge.
The nerfhammer has really hit you hard.

Healing Surge has become nearly obsolete nowadays (sadly).

In direct comparison to [spell=77472], (assuming [spell=51564], 5% Haste-buff, and the usual 916 Haste)

Healing surge has a cast-time of 1,33 seconds. It also has a high crit chance - 60% with 30% from your gear.
[spell=77472] has a cast-time of 1,71 seconds.

Due to a higher Base-healing, and a higher spellpower-coefficient, [spell=77472] will heal for a lot more.

So for making Healing Surge effective, you'd need a really urgent emergency-situation, or no tidal wave-stacks active.

Just because I like numbers and stuff:
Base Heal is 4802
Scales with 0,483% of your Spellpower (without [spell=16213] and [spell=84848])

In patch 4.0, its a heck of spell. Particularly while leveling. Its way faster then Healing Wave and only does a little less healing then Greater Healing Wave (which you don't even get till 68 after patch 4.0).

It is mana intensive, but considering the speed this is going be your go to direct healing spell. With 3/3 Tidal Wave Greater Healing Wave is reduced to about the same cast speed. GHW will heal for a little more, but will cost a little more mana. Plus you have to get a Chain Heal out first to get Tidal Wave.

So that makes Healing Surge a great general use heal. With this spell, 9 charges standard with Earth Shield, plus Cleansing Waters Shamans are much more resilient in 4.0.

I will go the step further say once you have sufficient haste, healing surge should not be used at all. it is a mana sink and can get you into deeper trouble in longer fights if you use it as your goto 'oh, S#%@' button.

Regarding your note to no tidal Waves stacks: in that case Healing surge is losing 30% crit chance - and no crit means Ancestral Awakening proc and no mana return (Resurgence). Ancestral Awakening is what makes Healing Surge CRIT heal just about the same amount as Greater Healing Wave HIT. This should also be considered.

In 6.0.2 and 6.0.3 (so far), Healing Surge is pretty much spammable for the whole fight. Heck i had 70% of my healing as HS and i was #2 on the healing chart on many occasions (close behind).

Using Riptide and Unleash Life before casting this empower it, so it's useful for topping someone up in a sec.

PvP multipliers are not in the tooltip or minable in the spell data.

From the July 26 2016 7.0.3 hotfix notes:

For Restoration Shamans, Healing Surge now heals for 20% more in PvP combat, Healing Wave heals for 35% more in PvP combat, and Healing Stream Totem heals for 25% more in PvP combat.

This is the shamans burst healing spell. Don't expect to be using this one if your enhancement unless you like being oom. Even elementals go oom fairly quick using this.

Spamming this as elemental can prevent wipes if healer dies, but only buys you 30 secs or so.
