Keep in mind you will not be able to use this skill if you are dead and the cooldown finishes while you are dead.
I like seeing the icon on my button bar, as well as a counter for my Anhks. As a passive ability, you can't drag it to your bar. However a simple macro can overcome this. It'll show a counter for your reagent, as well as the cooldown.

Tooltip
Allows you to resurrect yourself upon death with 20% health and mana. This effect can occur only once every 30 minutes.
While you are at full health, Reincarnation cools down 50% faster.
