4. The brimstone destroyer is not in a cave, you should be looking for a mountainpath which starts at 20.18, 19.6
A way to get there is to fly to Sylvan Falls, Highmountain, and follow the road south into Suramar. Just before you enter Suramar there is a small wooden bridge at 31.0, 80.5 Highmountain. Standing on the bridge look to the left and take the small path which is lead directly to the Brimstone Destroyer.
