you get him to spawn by killing a couple of the ghost mobs around the area
idk i think it was like 5 or 6? i lost count lol
To make him spawn, kill a bunch of Agonizing Shades in the area.
I've seen him respawn even when I saw his body from the previous spawn still on the ground - as long as you get the world quest in your quest list, you're golden.

For me it was 7 and the adds you kill are the grey ghosts called Agonizing Shade who are standing above the feared Unworthy Souls.
