You can get this quest from Sir Thomas after killing the Headless Horseman in the Scarlet Monastary instance during Hallow's End, you can then hand it in to the Masked Orphan Matron who is at one of the following three locations:
- 53.6, 41.2 - Razor Hill (Durotar)
- 60.9, 53 - Brill (Eastern Kingdoms)
- 47.3, 46.5 - Eversong Woods (Eastern Kingdoms)
This person called Thomas actually IS the Headless Horseman himself, only before he flipped out and started killing random people.
