TheWoWDBAt the base of the double-waterfall is an underwater cave. You have to go straight down between the falls and find the crevasse. Then its like most other underwater caves.

The World Quest Neltharion's Lair: Crystalline Crusher requires an optional boss to be killed in addition to the normal four bosses.
Ultanok is located in the cave called Neltarion's Lair. This is reached by going down in the waterfall (approx 66 86) after the boss Rokmora into an underground cave to kill Ultranok (at 70 87). Note that there is a treasure chest in the rear of that cave.
This World Quest can be completed in Normal, Heroic or Mythic modes. If you get locked out on the heroic raid and the group bypasses Neltharion, just requeue in any of the other levels, then kill Neltarion and you will be able to hand in the quest.

Here's a good video showing the crevice you need to dive through. The waterfall is at the end of the barrel run through the rapids. When your barrel hits the water below, stay in place and swim straight down to find the crevice. The cave is under the waterfall. The crevice can be seen at 0:35 seconds into the video. It looks like natural rock pulled away from a concrete formation.
