It looks like having completed the quest The Battle Plans before patch 4.0 stops you from obtaining and completing this quest.
Same thing for me, I was stuck at 43/45 with only chicken quest to be delivered to Booty Bay. Yes, the battle plans quest was bugged for me, I got no sequel. i decided to travel to BB. I got the ship from ratchet. When I was out of the ship, my counter was... "hotfixed" to 44/45! So delivering the last quest gave me the achievement.

What to do
Description
With the battle plans of the gnolls discovered, all that now remains is to bring the plans back to Hadoken Swiftstrider in Camp Mojache. Perhaps he or someone there can make further sense of what the gnolls are up to.
