In my observation, Hatefire spawns after 3 waves of infernal attacks.
This starts with a Demon Hunter npc yelling "Ready your blades! The demons come!" Then infernals will spawn. Each wave will spawn after the previous is dead. After the 3rd wave is dead the npc will yell "Bring down the monstrosity! Nothing gets through!" and then Hatefire will spawn several seconds after.TL;DR: Kill infernals to spawn Hatefire.

I waited for what seemed like nearly 20 minutes for Hatefire the Burning to spawn. He had not appeared, there wasn't even a corpse when I arrived and I was the only person doing this world quest. Just before I decided to give up and leave, he did finally spawn.
I am not sure if there is a trigger for him to spawn, or if he just respawns really slowly... but fear not, he will eventually spawn. Make sure you tag him quickly as he dies very fast, faster than most other 'named' quest mobs in the Broken Shore.
If this quest is as bugged as it appears to be, skipping it entirely may be a viable option. You only need 4 quests to complete the invasion, after all!
This worked for me. He spawned after about 5 minutes of killing everything in the area.
