This is Yogg-saron's Hardest Hard mode , if for some reason , your guild manages to do it FIRST on your realm , your e-peen wil grow by getting the title "<Name>, Death's Demise.
The Old Gods (including Yogg Saron) are based off the Cthulu Mythos, and this achievement very much references the roots of the entire WoW creation storyline as from H.P. Lovecraft's works.
