For this achievement you will have to transform an [npc=122532] into a [npc=123531]. To accomplish that, the Ember has to get hit twice by Aggramar's [spell=244688] combo.
The [spell=244688] is [spell=244291] - [spell=244033] - [spell=244291] - [spell=244033] - [spell=246014]. You can set focus on the [npc=122532] and check its debuffs to see if all abilities have hit.
Ideally, for this achievement you should bring a Warlock and a Demon Hunter, Druid or Warrior tank.
- First, let the [npc=122532] move away from the edge of the platform, and then have the Warlock cast [spell=710] to make it immune to damage. Make sure to keep the add crowd controlled throughout the fight.
- As soon as the tank gets knocked back by [spell=244033], they have to use [spell=189110], [spell=16979] or Charge on the boss to get back on the platform.
In case you don't have any Warlocks, you can use Freezing Trap, Entangling Roots or other crowd controls, but Demon Hunters will have to adjust where they use [spell=189110] to avoid killing the add.
Note that you have to transform the [npc=122532] within a single phase or else it will detonate.

Got this on live on December 1st. This achievement is fairly straightforward, but requires a fair amount of self-control from your raid.
In both his first and second intermissions at 80% and 40% respectively Aggramar will go immune to damage and summon small adds called Embers of Taeshalach, which will try to run to the boss, and explode when they reach him. In Normal killing these adds will remove them from the fight, but in Heroic they respawn after 18 seconds and must be allowed to reach the boss in a staggered fashion to not overwhelm the raid with damage due to a stacking damage taken increase for each Ember that hits.
For this achievement, you'll have to CC one of these adds (Banish if possible, Frost Trap/Entangling Root otherwise) and avoid killing it for a while. When the boss pushed back into P2 you must hit this add with the abilities from Aggramar's combo sequence twice. What this means is that once the add is CC'd, the tank should pull the boss near (not on top of) the add and face the boss toward it. When the boss casts Foe Breaker, tank 1 should be alone, and ensure the boss is close enough that the add is hit. Then the entire raid should stack in front of the boss to help split the Flame Rend. Then the entire raid should move and tank 2 should taunt, and take the second Foe Breaker alone with the add. Then the entire raid will stack up again for the second Flame Rend, and after this is over everyone in the raid needs to move out for Searing Tempest. The add needs to be hit by all 5 of these moves, twice each.
Each ability that hits the add will give him a stack of the respective buff, with the Ember becoming a Manifestation after 4 stacks of Foe Breaker, 4 stacks of Flame Rend and 2 stacks of Searing Tempest, or two full combos. Also if the add is alive for 3 minutes, it will explode regardless of it being near the boss, so there is a time restraint and you can't simply take as long as you need.
Once the Ember transforms you simply kill it. It gains a stacking raid-damage buff but it has virtually no health so this will never actually matter.
