Easy Solo at Level 70 Dragonflight, Level 70 Worldsoul Saga & Beyond...
1. Normal Difficulty.
2. Clear trash in Vectis boss room.
3. Head back out and pull Warmother Rakkali into Vectis boss room, Warmother is located down the ramp opposite to Vectis boss room entry tunnel.
4. Begin Fight.
5. Reduce boss health to just above half, slow down dps as you get closer.
6. Keep Warmother on you or your pet, and when Vectis launches a Plague Bomb be sure to stand in it with Warmother.
7. Warmother gets buffed up!
8. Kill Warmother.
9. Kill the Boss.
10. ?
11. Profit. Achievement Completed.

Defeat Vectis in Uldir after infecting Warmother Rakkali with Plague Bomb then defeating her on Normal difficulty or higher.

Warmother Rakkali is located down on the platform right before you go to Vectis.
After you get the Plague Bomb debuff on her, you can safely kill her, then kill the boss and you'll get the achievement.
