Ok, This is related in some way to the possibility of crafting 470 gear.
This is the full explanation
First, go to any of the Assault zones and kill mobs, from any random you will end looting Recipe: Void Focus [item=171312]
Continue killing mobs in assaults to loot 10x Void Focus Splinter [item=170553] , droprate is not a cake btw.Second, you will start a serie of quest.
After crafting Void Focus [item=171320] you will start the quest Something in Your Mind Something in Your Mind, just hand in.
Next two quest are to farm [item=171176]You will start with Maintaining Focus to loot x20 and later you will continue with Dream Catcher to loot x200
As the tooltip says Shred of Insanity can be looted from various activities ingame: from bosses in Ny'alotha and World Bosses, obtained by winning
Battlegrounds and Arenas, or from Honorable Kills.You will be limited in number by the quests.
Doing this quests you will be rewarded with items to use and upgrade your [item=171320]

This is beyond stupid. You need 200 x shred of insanity to craft a single item.
That means that the only decent way to get them is to do Raids on Mythic.
Explain me please, blizzard, why a Mythic raider will need to craft Mythic gear he is already getting.
The ones that need crafting 455+ or 470+ gear are those who can't raid. I don't mean to gift every profession crafter with almost free Mythic gear, but the actual requirements to craft this are to do something that someone that needs to do it can't do.
So a perfect catch-22. In essence: useless for non Mythic raiders.
It might not be so bad if this quest unlocked the upgraded Void Focus for all your characters, but they actually expect you to do this for EVERY alt with a crafting profession. That really is insanity!

Description
It appears that the focus remains stable, and its containment field remains intact. I daresay we can empower it even further. Hunt down and defeat formidable enemies to acquire additional remnants of N'Zoth's influence. This time, we will infuse the focus with even greater amounts of the Old God's power. I am confident that the containment field will continue to hold.
