If you have multiple quests that take you into the Halls of Origination, you may want to drop all but one and complete them before picking up another. Particularly Wrathion's "Deeper Into the Darkness" and High Explorer Dellorah's "A World Worth Saving." (Alternately, if the instance is working, complete whichever the game puts you in then working on the next quest.)

I had both Wrathion's and Dellorah's quests, teleported to HoO via MOTHER and backed out to do Dellorah's quest first. After finishing Dellorah's, I could teleport back via MOTHER but the quest instance was completely empty. No enemies, nothing to interact with and all doors out sealed off. Even after hearthing out and teleporting back via MOTHER, it remained this way. (If this happens to you, drop Wrathion's quest and it will kick you out of the instance.)

I had to drop and restart "Deeper Into the Darkness" to get the quest instance to work again.

    Protecting one's mind against the old gods requires an object of pure corruption. Unbeknownst to many, one such object exists--dormant beneath the molten pits of Blackwing Descent. Until recently, one of my agents stood watch over the area, keeping detailed records of suspicious activities.
