In case you are here because the NPC just despawned and you can't figure out what to do.. Just relog. Fixed it for me and Lady Moonberry appeared again.
If you are new to this covenant and she disappears after the cutscene, don't worry. For me, she reappeared with the ability to hand in this quest, after accepting the quest The Endless Forest.
Not being able to hand in this quest does not prevent progress at all, nor does it give any reward.
