I believe I have just found out a way to make Limbflayer spawn, hopefully for most people.

What worked for me, is that I had picked up the quest Trophy of Glory: Biolante, which is from Writhing Green Tendril that drops off of Biolante in Gorgrond.

After turning it in to Durotan in Beast Watch I was surprised to see Limbflayer standing in the Arena with a big ? over his head!

Oh, and I think I should mention that I had done all the previous Gorgrond Quests that I could, lore wise and "Bonus Quests" wise, too.
