You need to go to the Vault room which is northeast of the quest giver at the bottom of the cave. There are stones on the walls. Each time you click on one, a mob appears. After you kill the mob, it drops a Eye of a Bleeding Hollow Chieftain, which is a grey item, but pick it up. Keep clicking on the stones in the wall and collecting Eye of a Bleeding Hollow Chieftain until you get notification that the quest is completed and you found the Eye of Kilrogg. (I got it on my 6th stone). Your quest companion Ariok will hold back the rest of the mobs that spawn. Meanwhile, you should go to the Ritual room next door, which is directly west of the quest giver.
Pick up the object on the ground in the middle of the circle. After some dialog, you'll fight the Eye of Kilrogg. Ariok is the turn in NPC, and he is waiting for you on the road back upstairs.

I kept going to each skull, opening it, seeing it wasn't 'Kilrogg's Eye' and then closing the loot window without taking 'Eye of the Bleeding Hollow Chieftain'. I must have been doing this for 25 minutes before realising you had to pick up five of those first.
After going back to my garrison afterwards I am horrified to discover dozens and dozens (maybe hundreds?) of those eyes in my mailbox, having not looted them...

What to do
Description
If you wish to know what Kilrogg and his new master intend, you will need his eye. After the ritual all chieftains place their eye under the protection of the ghosts of the Bleeding Hollow in the next chamber. Go and locate the Eye of Kilrogg in the vault. The other chieftains will not stand with us, and will fight you for it. Then place it on the ritual mark nearby to get your answers.
