Another one that can be done solo by stealth classes.
When you enter, walk down the first set of stairs, take a left or right (doesn't matter). After walking around the first mobs, watch out for the small spiders. They are easy to miss since they look like the critters that you see in almost every instance, but there are a lot of them and they can kill you.
Then you just keep walking around and down to get to the room with the quest item. There are two large voidwalkers that walk into and just outside of the room. I waited until they stepped out and was able to walk in behind them, loot the item, and re-stealth before they came back.
Then just hearth or stealth back out the way you came in.

Even though it only lists [item=118651] as the requirement, you must also kill Ner'zhul in order to turn the quest in.
The original comment predates the hotfix that made killing the last boss of the instance a prequisite for the quest.

Description
My 'master' is an idiot, but you look like you've got some smarts in that oddly-shaped head. Listen up, the Twisting Nether is a chaotic abyss, and things work differently there. I've felt some disturbances, and they don't bode well for any of us. Yes, there are things happening here, in Draenor, that are causing ripples in the nether. No, don't try to wrap your head around how that works, you'll only hurt yourself. It's simple: bring me a Void-Gate Key, and maybe I can stop the bad things.
Getting there

Bosses in the Adventure Guide, in order: Sadana Bloodfury, Nhallish, Bonemaw, Ner'zhul.
Entrance: Terrace of the Stars, Shadowmoon Valley at 31.8, 42.7.
Instance text, bosses and entrances are the client’s Adventure Guide data.
