This is a new achievement in patch 5.4 that you get for kicking Garrosh's ass as an Alliance player on normal or heroic difficulty in the Siege of Orgrimmar. It awards the title 'Conquer of Orgrimmar' .
The Horde version for this achievement is "Liberator of Orgrimmar".
If you complete this on heroic you also get : Hellscream's Downfall

Defeat Garrosh Hellscream in Siege of Orgrimmar on Heroic or Mythic difficulty.

I don't think you can get this with LFR, but correct me if I'm wrong.
