Its not inside the tomb. You have to follow the coast to the west point where you can use the grapple to get to him.

For those without coordinates or prefer not to use them, if you circle around to the west side of the island, you'll see a Sea Skrog sitting down in front of a bunch of crabs. The grappling hook point is just south of that.
