Drops off the first mob and you did not notice it in your bag. I had lots of quests throughout the world start like this as loot from the first mob so we have a reason to stay and get more experience points from the quest. An addon helped me see the quest quickly in case I forgot it was in my bag.

3 quests in Hillsbrad Foothills appears to have a level requirement higher than 15 to be initiated.

On a XP off character locked at level 15, I spent some time trying to get these quests to start, but without success. Once I came back, locked at level 21, the quests started instantaneously.

This might explain some comments about leaving the area and coming back later fixing problems, as they probably leveled up in this time.
