If you come into this area via an alternate route, you may encounter an NPC that gives Skulltaker's Revenge. If you pick up this quest and complete it, you canNOT see the NPC to turn in quest until you start Basic Skulltaking, which phases the area correctly for you. I reported this to Blizzard, but with all the other problems, I don't know that this would get fixed. Just go get Basic Skulltaking first to phase the area.

Description

The others... gone or wandered into the haze. It called to me as well but I am too strong. I chose to kill until they crushed my body. I haven't killed enough of them yet {name}, not enough! Go. Kill more of them and tell my hunt leader that I don't go alone into the dark.
