The rares come around fast enough for people to tag them ASAP. So, if you happen to find a rare (or places where a few rares spawn) then it might be best to just hang around in that area until respawn. If you spend your time running around hoping to find one that just showed up, odds are 20+ people have beat you to him.
Now, if you can get a few hits on the mob before they die, you can still loot. However, there's normally a good mix of horde/alliance coming together to bring the rare down and you might miss your chance to loot if the opposite faction decides to clear out anything red.
A good place to camp is around 44,84 on the island. This area has about 4 rares within a short distance.
The names and APPROXIMATE coords are:
[npc=72909] 40, 82 (surrounded by a circle of smaller worms)
[npc=72193] 34, 85 (off the dock, very large beast)
[npc=71919] 37, 77 (middle of nearby town, will spawn when 10 [npc=71908] are killed)

Description
An island lost in time and yet it returns flourishing with life! Where did this island travel to? Did it simply hop forward in time, or did it languish in the dark reaches of the Twisting Nether? There is one thing for certain: the creatures that inhabit this place are far more than they seem. I sense a hidden power in them, forged by the eons. Seek out the champions of their species, duly marked by their power and prowess, and vanquish five such creatures. In death, we may find the clues we seek.
