The quest description claims you can get the quest items (shreds of insanity) from world bosses and winning battlegrounds. Well, after doing a few of both, I haven't gotten a single one.
Got 1 from each wing of the Ny'alotha raid, though, lfr difficulty.
Allows you to craft iLvL 455 gear from the new 8.3 recipes. The prerequisite quest is Something in Your Mind.

Description
At present, the focus holds but a fraction of N'Zoth's true power. If we further empower it, we could craft even more effective weapons to wield against him. N'Zoth's influence is everywhere, {name}, and is most concentrated within powerful foes. Vanquish them and claim whatever vestiges of the Old God's power remain.
