currently bugged. keep abandoning it and retrying and eventually it will work.
two tips:
-accept the quest, abandon it, and relog
-walk forward initially, but then stay behind her the whole time. she is slow, if she goes fast, you bugged it
-get off your mount and just walk (while staying behind her. DO NOT FLY!)

she will say 'reveal yourself, mal'ganis' and the main dredger will transform. if she doesn't do this, abandon and restart.
5 seconds later, the 4 dredgers around him will transform. if they dont do this, abandon and restart
took me about 8 times to get it to work

The key thing here that worked for me was to start walking immediately after you accept the quest. She doesn't seem to move until you move and if you wait, then she'll start running and it bugs out. Try accepting the quest and then immediately start moving to get her started.

Description

The Medallion of Wrath is more than just a powerful artifact. In the hands of a nathrezim it could be used to shape this realm to their dark purpose. What is more, while they hold it we are denied the ability to forge a new sigil. They already nearly destroyed the Ember Ward with their reckless schemes. I will not have my medallion used for their ends. Come, let us get it back.
