Even though you can pick up this quest while phase diving, the objectives are in the normal world.

Be sure to be unphased for this quest as the objectives of the quest are in the normal world.

[npc=231164]s seem to drop the quest item more frequently than [npc=231167]s, because they don't seem to always drop loot.

Worth noting that the Hollow Reflections that drop the quest item have a buff on them that explodes their corpse very shortly after death. You need to loot fast as the bits that are left after are unlootable.
