This is a hell of corpse run to get back to Kleia (I was swamped by elites at the end of the quest). You respawn outside of Peredition Hold, and if you can't stealth, getting back to the questgiver is very difficult. You will also lose a lot of stygia in the process. Awful quest design.

Uther disappeared while I was reading the quest text. No Uther means I can no longer accept the quest. Cannot leave. Map shows where Uther is supposed to be but he is not there. Kleia and the sword will no longer help me kill anything, I'm exposed in an area where I've not advanced to yet, so I'm taking 200% damage in here. Well designed Blizzard.
Edit: After dying, I could not even re-enter Perdition Hold by myself, luckily some other Kyrian came with the quest, I followed them and got the quest when their Uther spawned.
Previous Quest: Her Will, Inflicted.
After picking up the quest from Uther you will need to turn around and talk to [npc=156361] who will fly you to the waystone (this is a requirement and can't be skipped). Once you arrive at the Waystone you will have to use it in order to advance. After arriving in Oribos you can hearth / fly to Elyian Hold.
Note: Do NOT hearth or teleport out. Talk to Kleia to progress to next step. The same is when you arrive at the Waystone and you must use it.

I knew that Devos had powerful allies. I turned a blind eye. When Lysonia took her place... I was blinded by my loyalty. I have much to reflect on. We may not be allies, but I have one request. Please, escort me from this wretched place. I will leave you be... but do not leave me here in the Maw.
