It's counting the hood you are wearing and when you turn it in, the NPC will take the gear you're wearing!
I had made a few and forgotten I disenchanted all but the epic upgraded one which I put on. When I turned in the quest, the NPC took my epic hood right off my head.
I put in a ticket and was told that "technically, what happened was intended" because there wasn't one in my bags.
This gets turned in to Warmage Silva just outside the door to Violet Citadel.
There is a bug with this where it shows up as completed on your map, but it is not completed. Just double-check if you have created the silkweave hood.
