I had made a few and forgotten I disenchanted all but the epic upgraded one which I put on. When I turned in the quest, the NPC took my epic hood right off my head.
I put in a ticket and was told that "technically, what happened was intended" because there wasn't one in my bags.

There is a bug with this where it shows up as completed on your map, but it is not completed. Just double-check if you have created the silkweave hood.
